Fast fix: disable the old SEO output, clear page cache/CDN, then recheck source.
The second breakage is sitemap caching. I’ve seen Cloudflare, server cache, or a security layer cache old sitemap responses or block them outright. The quickest test is a direct request to the sitemap URL and checking for a real 200 response. If it fails, exclude sitemap URLs from cache and make sure no security rule is rewriting XML responses.
